Job Summary

Coordinate holistic long-term care for Physical Disability & Aging Waiver members by assessing needs, recommending and maintaining service care plans, and connecting members with providers and community resources. Provide education to members and caregivers, monitor care plans and outcomes, and document member information to ensure compliance with state, federal, and payer requirements. This hybrid-remote role includes 50–75% local travel for home and supportive living visits.

•Evaluate member needs and available resources to recommend and facilitate the best outcome.
•Assist in developing and maintaining long-term care service care plans and identify needed providers and community resources.
•Coordinate between members/caregivers and the care provider team to ensure services are accessible.
•Provide resource support across needs such as employment and housing, and monitor plan outcomes to update recommendations.
•Conduct on-site visits as needed and provide education to members and caregivers on services, benefits, and procedures.

Key Requirements

•Bachelor’s degree and 1 year of related experience.
•For the Illinois Physically Disabled/Elderly track: hold an Illinois RN license, or a Bachelor’s/Master’s prepared in a human services related field, or an LPN with 1 year experience conducting comprehensive assessments for the elderly.
•Experience with waiver, case management, advocacy, or home visits/community travel (preferred).
•Ability to complete long-term care assessments, develop service care plans, and coordinate needed providers and resources.
•Ability to collect, document, and maintain member information and care management activities for compliance.
