Key Responsibilities

  • •Coordinate care of individual clients using assessment, care planning, implementation, coordination, monitoring, and evaluation for cost-effective, quality outcomes.
  • •Provide intensive behavioral health care coordination and conduct comprehensive health risk and needs assessments across psychosocial, physical, medical, behavioral, environmental, and financial parameters.
  • •Develop and communicate care plans as the point of contact to ensure services are rendered appropriately during transitions and through community-based services.
  • •Implement, monitor, and measure strategies and interventions to improve health and quality of life outcomes, including ongoing review to identify gaps and trends.
  • •Collaborate with interdisciplinary care plan teams and maintain professional relationships with inpatient, outpatient, and community stakeholders; generate reports and adjust plans as needed.

Key Requirements

  • •3-5 years of experience in Social Work, Nursing, or a healthcare-related field (or relevant experience in lieu of degree).
