Care Coordinator - Portales /Clovis, NM

Coordinate care for behavioral health and other member needs across the Portales and Clovis communities, using assessment, care planning, implementation, monitoring, and evaluation. Serve as a point of contact to ensure appropriate services during transitions and across home/community settings. Conduct health risk and needs assessments, document and maintain enrollee records, collaborate with interdisciplinary care plan teams, and generate reports to improve outcomes and member satisfaction.
