Maintain an ongoing caseload, use relationship-based engagement strategies, and perform telephonic and SMS outreach to ensure timely follow-up. Collaborate with an interdisciplinary care team, support Health Action Plan/Shared Care Plan goals, and connect members to community resources while meeting encounter metrics. Remote-forward role with 2–3 monthly community on-site visits.
