Case Manager

Manage clinical documentation and coding to support medical necessity, accurate risk adjustment, and optimal case mix. Review outpatient records against E&M documentation guidelines, ensure correct coding to prevent down-coding and revenue leakage, and analyze insurance claim rejections and denials through audits. Educate physicians and departments with training modules to improve documentation quality and billing accuracy, while also supporting OSHMS/EHSMS compliance responsibilities.
