Home Based Primary Care Physician (MD/DO)

Provide relationship-centered, outcomes-driven primary care to Medicare Advantage members in their homes across New Jersey. As a field-based staff physician, you’ll manage a defined panel with complex medical, functional, and social needs, conduct in-home assessments, lead post-discharge transitional visits, and proactively intervene when conditions change. You’ll coordinate care with an interdisciplinary team and use Clover Assistant and other clinical technology to surface risks and close gaps.
