Key Responsibilities

  • •Assess and address health and social service needs using peer coaching strategies, including health education, friendly check-ins, and resource linking by phone.
  • •Use community resources, social services, and partnerships to meet needs, communicate questions/concerns to staff, and maintain appropriate conversation topics and rapport.

Key Requirements

  • •Must be located in Santa Clara County and meet one of the California CHW minimum qualification pathways (certificate pathway or work experience pathway).
  • •Certificate pathway: completion of a curriculum with demonstrated training in communication, interpersonal relationship building, service coordination/navigation, outreach/evaluation, and public health/SDOH knowledge.
  • •Work experience pathway: at least 2,000 hours working as a CHW in paid or volunteer positions within the previous three years with demonstrated skills in the required areas.
  • •Reliable transportation and a flexible schedule for face-to-face outreach, plus excellent written and verbal communication skills (in person, via phone, and video conference).
  • •Strong computer skills to learn and operate web-based systems, with HIPAA confidentiality, and high organization and attention to detail.
