Key Responsibilities

  • •Drive program engagement by communicating the value of Maven’s care coaching model and optimizing opt-in rates to support long-term behavior change.
  • •Co-create SMART goals with members and own progress by tracking adherence, adjusting care plans, and course-correcting within the clinical program design.
  • •Manage a caseload through ongoing messaging, video appointments, and outreach while adhering to clinical guidelines and protocols.
  • •Provide personalized health education by assessing member knowledge and barriers and using motivational interviewing and behavior modification techniques.
  • •Monitor member health data and platform activity remotely to identify risks early, coordinate next steps with providers, and follow established protocols.

Key Requirements

  • •1-2+ years experience in health coaching or clinical work (e.g., nursing, behavioral health), including patient education and motivational interviewing.
  • •Experience coaching maternity populations and/or individuals navigating family planning or fertility care.
  • •Training and experience coaching individuals to achieve specific health outcomes (e.g., medication management, nutrition and exercise goals, managing blood pressure).
  • •Ability to communicate empathetically in writing and verbally and drive positive member experience with optimal outcomes.
  • •Comfort learning and adapting to new digital health tools; strong telehealth competency with phone outreach and video calls.
