Key Responsibilities

  • •Review your case load each day and prepare the right technology and tools to service patients.
  • •Navigate your daily schedule using modern technology and coordinate with in-home clinical team members (Sprinters).
  • •Conduct thorough wellness exams including health risk assessments, medication reviews, and cognitive screenings.
  • •Work with Sprinters to evaluate vital signs, arrange blood draws, and support patient-specific needs.
  • •Provide care coordination and management, including preventive care interventions, medication management, specialist referrals, community resources, and documentation of findings.

Key Requirements

  • •Board Certified as a Family Nurse Practitioner.
  • •Active Family Nurse Practitioner license in Louisiana.
  • •Willingness to work in a fast-moving environment that may require last-minute problem-solving.
  • •Technologically comfortable with laptops/mobile devices for charting and HIPAA-secure messaging apps for care coordination.
  • •Strong written and verbal communication skills, with ability to work independently or in a team environment; pass national background check and valid clinical license search.
