Nurse Practitioner (Remote, NJ License Required)

Provide phone and video-based follow-up care for patients in the Transitions of Care program after hospital discharge. Conduct telephonic outreach and scheduled visits, performing medication reconciliation and symptom screening while ordering needed follow-up labs, medications, and supplies. Coordinate care across hospital/ED, pharmacies, and community providers, collaborate with nursing teams, and work toward individual performance targets and population health goals within a digital-first, 24/7 organization.
