Key Responsibilities

  • •Identify, outreach, engage, and enroll eligible individuals into the program via phone, text, email, and in-person at partner locations.
  • •Listen to members’ needs, adapt outreach approaches, and build trust to overcome hesitancy toward healthcare enrollment.
  • •Educate partners on Pair Team’s services and link eligible members to Enhanced Care Management.
  • •Explain Enhanced Care Management services and benefits while supporting members through the enrollment process.
  • •Manage the end-to-end enrollment funnel, providing timely updates on funnel stages, delays, and blockers while meeting enrollment metrics.
