Key Responsibilities

  • •Assist with developing and coordinating holistic care management activities and personalized LTSS service care plans.
  • •Identify and coordinate providers, specialists, and community resources needed for long-term care members.
  • •Coordinate between members/caregivers and the care provider team to ensure services are accessible.
  • •Monitor care plans/service plans, member status, and outcomes; provide recommendations based on member needs.
  • •Collect, document, and maintain member information and care management activities to ensure compliance; may perform home or site visits and provide education.

Key Requirements

  • •Bachelor’s degree and 2–4 years of related experience, or an equivalent combination of education and qualifying experience.
  • •Case management experience for the aged, including behavioral health conditions, or for people with physical/developmental disabilities, or HIV/AIDS population.
  • •Must reside in Orange or Seminole County, Florida.
  • •Experience with assessments and care planning for long-term care members.
  • •Bilingual (Spanish) and strong communication and customer service skills.
