Job Summary

Coordinate holistic long-term services and supports care management for physically disabled and elderly waiver members. Assess needs and resources, develop and monitor individualized service care plans, and connect members and caregivers with providers and community resources. Use documentation and follow compliance requirements with state, federal, and payer regulations. This hybrid-remote role includes 75% local home visits across a designated Illinois region, supporting quality, cost-effective care outcomes.

Key Responsibilities

•Assess member needs and available resources, and recommend and/or facilitate the best outcome.
•Develop and monitor long-term care service care plans, identify needed providers and community resources, and update recommendations as needs change.
•Coordinate between members, families/caregivers, and the care provider team to ensure services are accessible.
•Provide resource support across needs such as employment, housing, independent living, and related services based on assessments and plans.
•Perform on-site/home visits as needed, educate members and caregivers on services and benefits, and maintain required documentation for compliance.

Travel: High travel

Salary: USD 22.94 - 38.79 hourly

Key Requirements

•Bachelor's degree and 1 year of related experience.
•For Illinois Plan: be RN licensed in Illinois, or have a bachelor’s/master’s in a human services related field, or be an LPN with 1 year experience providing formal service for the elderly.
•Experience with case management, advocacy, or home visits/community travel is preferred.
•Ability to evaluate member needs and recommend and/or facilitate service plans.
•Ability to collect, document, and maintain member information to ensure compliance with applicable regulations.
