Key Responsibilities

  • •Support care management activities and assigned member teams to ensure services are delivered and continuity of care/member satisfaction is achieved.
  • •Provide member outreach via phone or home visits and document care/service plan activities and next steps.
  • •Coordinate care activities based on the care plan/service plan with healthcare and community providers, partners, and members/caregivers.
  • •Serve as support for member and provider inquiries, requests, or concerns related to care plan/service plan, and communicate with care managers and practitioners.
  • •Document and maintain member records per state/regulatory requirements and assist with assessments/screenings and referrals as needed.
