Job Function: Administration & Executive Assistance

Key Responsibilities

  • •Actively locate, contact, and engage clients who have been identified as eligible for CHW services, in the community or provider locations.
  • •Use motivational interviewing, trauma‑informed, and harm‑reduction approaches to support clients in accessing healthcare and navigating the healthcare system.
  • •Lead or help facilitate community workshops and group activities on topics such as health education, relationships, nutrition, stress management, and housing resources.
  • •Provide trauma‑informed care coordination and referrals, connecting clients to housing resources, transportation, and public benefits.
  • •Maintain accurate and up‑to‑date records of client interactions, care plans, and referrals in the EHR system.

Pay and Benefits

Salary: USD 26.5 hourly

Key Requirements

  • •Bachelor’s degree in Social Work, Psychology, Nursing, or a related field; OR Associate’s degree in Social Work, Psychology, or related field with at least 2 years of mental health or social services experience; OR equivalent combination of training and professional experience.
  • •Minimum of 2 years of experience in case management, care coordination, or related social services role.
  • •Certified Community Health Worker (or willingness to obtain certification within a designated timeframe).
  • •Strong cultural competency and sensitivity to the needs of diverse populations.
  • •Proficient verbal and written English communication skills; bilingual or multilingual in Dari or Pashto preferred
