Provide hands-on home care as a Certified Medical Assistant within the Clover Care Visit team. Partner with Providers before, during, and after visits to help patients through translation, vitals, questions, consent form review, and accurate documentation. Perform phlebotomy and other essential tasks as directed, coordinate via EHR/EMR systems, support care-gap closure and outreach scheduling, and travel independently for member visits in Carroll County, GA.

•Basic computer proficiency and comfort documenting in electronic medical records (EMRs).
