Key Responsibilities

  • •Facilitate in-room telemedicine visits and schedule acute, follow-up, and routine provider appointments
  • •Prepare and support residents during provider visits, including positioning and documentation, and update EHRs with medical histories
  • •Support transitions of care across the post-acute continuum, including referrals, follow-up appointments, and prior authorizations
  • •Reinforce provider instructions by educating residents on nutrition, fall prevention, medication reminders, and general wellness; distribute approved materials and route concerns
  • •Maintain accurate EMR documentation and support regulatory compliance/quality initiatives; assist with discharge planning and coordinate transition-of-care visits
Travel: Low travel

Key Requirements

  • •Graduate of an accredited Medical Assistant (MA) program
  • •Certified Medical Assistant (CMA) preferred
  • •Prior Medical Assistant experience in a clinical or care-coordination setting (health plan/hospice liaison experience preferred)
  • •Experience charting in an EMR
  • •Knowledge of geriatric medical practice and terminology; strong communication and interpersonal skills
