Key Responsibilities

  • •Evaluate member needs, barriers to care, and available resources, then recommend and facilitate plans for best outcomes.
  • •Develop or contribute to personalized care plans/service plans and identify required providers, specialists, and community resources.
  • •Provide psychosocial and resource support to members/caregivers to access local services and supports such as housing, employment, education, food, independent living, and foster care-related resources.
  • •Coordinate between members/families/caregivers and the care provider team to ensure timely access to care and services; monitor progress and collaborate on care plan revisions as needed.
  • •Collect, document, and maintain member information and care management activities to ensure compliance with current state, federal, and third-party payer regulators.

Key Requirements

  • •Bachelor’s degree and 2–4 years of related experience.
  • •Graduate from an accredited school of nursing if holding clinical licensure.
  • •For Sunflower Foster Care: at least a bachelor’s degree in social work, nursing, psychology, or related health and human services, or be an RN with at least one year coordinating services across child welfare and related systems of care.
  • •Current state’s clinical license preferred.
  • •Must reside in the state of Kansas for travel up to 75%.
