Key Responsibilities

  • •Cover care needs for the Transitions of Care program.
  • •Conduct telephonic outreach and scheduled video/phone follow-up visits for recently hospitalized and discharged patients.
  • •Perform medication reconciliation, symptom screening, and order needed follow-up labs, medications, or supplies.
  • •Navigate conversations to build trust, explain the care model, and connect patients to services.
  • •Coordinate care across settings (hospital and ED transitions, pharmacies, and community providers) and collaborate with the nursing team.

Key Requirements

  • •Certified Nurse Practitioner in Adult, Family or Geriatrics.
  • •Active state licensure in SC, with the ability to obtain additional state licensures.
  • •5+ years of post-graduate clinical experience in Primary Care or Geriatrics.
  • •Direct experience engaging Medicare and Medicaid populations with complex, comorbid conditions.
