Key Responsibilities

  • •Review caseload and prepare technology and tools to service patients.
  • •Conduct virtual wellness visits and proactive care activities including health risk assessments, medication reviews, and cognitive screenings.
  • •Provide comprehensive care coordination and management, including preventive care interventions, medication management, referrals, community resources, and documentation.
  • •Collaborate with in-home clinical team members to evaluate vital signs and arrange blood draws as needed.
  • •Manage daily scheduling and partner effectively while working independently and maintaining quality standards.
