LTSS Service Care Coordinator

Coordinate holistic care management for long-term support services members with physical disability and aging waiver needs. Assess member needs and resources, develop and monitor personalized service plans, and connect members and families/caregivers to providers, specialists, and community resources. Maintain accurate documentation to ensure compliance with state, federal, and payer requirements, and provide education while performing a mix of remote work and 75% local home visits across southern Illinois.
