Key Responsibilities

  • •Review your case load and prepare technology and tools for patients.
  • •Conduct virtual adult and/or pediatric wellness visits including health risk assessments, medication reviews, and cognitive screenings.
  • •Provide patient education to support preventive care and well-being.
  • •Coordinate care directly and work with in-home Sprinters to evaluate vital signs and arrange blood draws.
  • •Manage care coordination tasks including medication management, referrals, community resources, and documentation of findings.

  • •Board Certified as a Family Nurse Practitioner.
  • •Active Family Nurse Practitioner license in Oklahoma.
