Key Responsibilities

  • •Provide evidence-based behavioral health interventions via the Collaborative Care Model to assigned patients based on care pod needs.
  • •Coordinate with primary care providers and other services as appropriate, tailoring treatment plans to individual patient needs.
  • •Screen patients for behavioral health and substance use disorders using tools such as PHQ-9, GAD-7, and suicide risk assessments.
  • •Monitor clinical symptoms and treatment side effects, re-engage patients using a registry, and support follow-up outcomes tracking.
  • •Document patient progress and recommendations in Salesforce and the electronic health record, and participate in consultations and supervision sessions.

Key Requirements

  • •Master’s Degree in Social Work, Counseling, or related field from an accredited program.
  • •Licensed to practice in Washington as an associate clinician under supervision (ASW, APCC, or equivalent) OR as an independent clinician (LCSW, LPCC, or equivalent).
  • •Minimum 1-5 years experience as a licensed professional.
  • •A secure, high-speed internet connection capable of supporting video conferencing and cloud-based systems.
  • •Proficiency with behavioral health screening tools and evidence-based treatment approaches (or willingness to apply them).
