Job Summary

Support care management by coordinating services for members to ensure continuity of care and member satisfaction. Conduct member outreach via phone or home visits, document care/service plans, and coordinate with healthcare and community providers to accommodate changes in progress. Help respond to member and provider questions, assist with service assessments/screenings, and maintain member records in line with state and regulatory requirements.

•Communicate with care managers, practitioners, and others to facilitate member services and maintain continuity of care.
•Support service assessments/screenings as needed, document care needs, and maintain member records in accordance with state and regulatory requirements.

Key Requirements

•High school diploma or GED.
•1–2 years of related experience.
•For the Arkansas Total Care plan: a bachelor’s degree in a social science/health-related field, or high school diploma with at least one year coordinating care for developmentally/intellectually disabled or behavioral health clients.
•Must have a driver’s license and complete child and adult maltreatment checks and a drug screen (before hire and recurring).
•Must reside in Arkansas (or a border city), and preferred candidates should have strong experience working with Behavioral Health and Developmental Disabilities (BH/DD) populations.
