Key Responsibilities

  • •Serve as the central point of contact for WTCHP CCEs, ensuring clear communication, timely issue resolution, and program alignment.
  • •Develop and maintain executive-level relationships with CCE leadership to support transparency and long-term partnership success.
  • •Act as the internal subject matter expert for CCE-specific workflows, including case management models and member coordination processes.
  • •Coordinate internal execution of CCE needs with cross-functional teams including Benefits Administration, Member Services, Claims, Provider Network Management, and Operations leadership.
  • •Facilitate business reviews, manage escalations, and provide CCE-focused insights, data, and reporting aligned with CDC and WTCHP requirements.

Key Requirements

  • •Bachelor’s degree with 5+ years of related experience (work experience considered in lieu of degree).
  • •Functional knowledge of all eight CCEs, including structures, workflows, and operational practices.
  • •Experience supporting or collaborating with federal healthcare programs, including familiarity with CDC program standards and compliance expectations.
  • •Proven ability to manage executive-level provider relationships within large academic medical centers or integrated health systems.
  • •Strong operational coordination skills and excellent communication, interpersonal, and problem-solving abilities, including managing escalations across cross-functional teams.
