Key Responsibilities

  • •Review daily patient census reports and partner with hospital teams to identify and prioritize eligible Medi-Cal and Medicare patients.
  • •Conduct in-person bedside outreach in emergency departments, observation units, triage areas, and inpatient units.
  • •Verify patient eligibility and educate patients and families about Enhanced Care Management.
  • •Enroll eligible and consenting patients before discharge whenever possible, and document outreach attempts, patient status, and enrollment activity in the electronic medical record.
  • •Coordinate warm handoffs and communicate pending follow-up needs to Pair Team and hospital partners, escalating workflow barriers as needed.

Key Requirements

  • •Bilingual fluency in English and Spanish.
  • •1+ year of experience in patient outreach, community health, enrollment, patient navigation, care coordination, or a related healthcare setting.
  • •Ability to communicate clearly with patients and families and build trust across diverse cultural and socioeconomic backgrounds.
  • •Comfort approaching patients in person, working independently in a fast-paced hospital environment, and managing documentation.
