Key Responsibilities

  • •Review your case load, prepare technology, and navigate your schedule to support virtual patient visits from home.
  • •Conduct wellness exams including health risk assessments, medication reviews, and cognitive screenings.
  • •Coordinate patient care by arranging blood draws, evaluating vital signs, and partnering with in-home team members.
  • •Manage care coordination tasks including preventive interventions, medication management, referrals, and community resources.
  • •Document findings and communicate securely using HIPAA messaging and other required tools.

Key Requirements

  • •Board Certified as a Family Nurse Practitioner.
  • •Active Family Nurse Practitioner License in California.
  • •Credentialed Medicaid provider for the state of California (Medi-Cal).
  • •Consistently demonstrate professionalism, integrity, accountability, confidentiality, care, and compassion.
  • •Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA secure messaging apps.
