Bilingual Case Management Specialist (Remote Flexible, Spanish Speaking)

Provide whole-person case management for individuals with serious mental illness, substance use disorder, homelessness, and other high medical needs. Maintain an ongoing caseload, conduct telephonic and SMS outreach, and help members set and achieve health goals via Health Action Plans and Shared Care Plans. Coordinate care with the Enhanced Care Management team and external/community supports, meeting encounter metrics while supporting a multidisciplinary model.
