Job Summary

Determine and evaluate social factors related to patients’ illness, hospitalization, diagnosis, and recommended treatment, and communicate them to medical staff to support integrated care. Provide confidential social work services to patients and families, including interviewing, planning services, and educating patients/families per their needs. Coordinate with medical staff and community resources, maintain case records, and support ongoing social service program planning.

Key Responsibilities

•Ensure superior quality of medical treatment, compassion, and understanding is given to every patient served at NMC Hospitals and Clinics.
•Interview patients to establish and assess problems, causes, components, and impacts, and determine present and potential consequences.
•Develop initial service plans, prepare and maintain case files, and prepare confidential records with proper access control.
•Provide assistance and services by referring to medical/community resources and supporting patients and families in modifying behavior patterns when appropriate.
•Educate patients/families according to their needs and implement patient/family education documentation and patient care planning, and consult with medical staff about social factors for integrated treatment programs.

Key Requirements

•Bachelor’s degree in social services.
•Two (2) years of professional social work experience under professional supervision, preferably in healthcare services or large-scale hospitals.
•Registration in SCFHS.
•Exhibits professionalism and excellent interpersonal communication skills.
•Excellent command of oral and written English and working knowledge of computer skills.
