Medical Assistant (Temporary Position)

Coordinate patient and provider visits in the skilled nursing facility setting by facilitating in-room telemedicine, scheduling appointments, preparing residents for visits, and maintaining accurate EMR documentation. Support transitions of care across the post-acute continuum, including referrals, follow-ups, prior authorizations, and education on nutrition, fall prevention, and medication reminders. Serve as a liaison between residents, families, providers, and nursing staff to support value-based care outcomes.
