Key Responsibilities

  • •Conduct longitudinal care management assessments and develop, maintain, and risk-stratify patient-centered care plans for an active caseload.
  • •Deliver individualized education and self-management support using Motivational Interviewing (MI) techniques and advocate across the care continuum.
  • •Execute episodic and transitional care management outreach after inpatient discharges and ED encounters, including post-discharge medication reconciliation and follow-up visit scheduling.
  • •Partner with primary care physicians and care teams to identify high-risk patients, lead case reviews, and use CRISP ENS alerts and Aledade App to monitor high-utilizers.
  • •Provide clinical oversight to non-licensed support staff, support population health initiatives, and monitor performance against clinical, financial, and functional quality metrics aligned to MDPCP standards.

Key Requirements

  • •Current active licensed Registered Nurse in Maryland.
  • •3–5 years of direct healthcare experience, preferably in home health, ambulatory care, community public health, case management, or care coordination across multiple settings.
  • •Experience providing care management across longitudinal and episodic/transition care, including post-discharge medication reconciliation and follow-up scheduling.
  • •Demonstrated use of Motivational Interviewing (MI) techniques with exceptional written and oral communication to drive engagement and health behavior change.
  • •Understanding of quality metrics and familiarity with health information exchanges (HIE), specifically CRISP ENS alerts, and population health management tools (Aledade App).
