Key Responsibilities

  • •Develop, assess, and coordinate holistic care management activities to support quality, cost-effective healthcare outcomes.
  • •Assist with creating and maintaining personalized long-term care service plans and service care plans based on member assessments.
  • •Coordinate between members/families/caregivers and care provider teams to ensure identified services are accessible.
  • •Provide resource support and education to members and caregivers, including procedures, service options, referrals, and healthcare benefits.
  • •Collect, document, and maintain member information and care management activities to ensure compliance with relevant regulators; monitor member status and recommend plan updates as needed.

Key Requirements

  • •Bachelor’s degree and 2–4 years of related experience.
  • •Or a Bachelor’s degree in health, human, social work, or education services with one or more years of qualifying experience; or a high school degree with three years of qualifying case management experience (aged, behavioral health, physical/developmental disabilities, or HIV/AIDS population).
  • •Knowledge of HCBS waiver and long-term care service planning.
  • •Strong communication and customer service skills with time management.
  • •Ability to perform assessments and care planning and coordinate resources and referrals.
