Bilingual Healthcare Call Center Representative (Spanish Speaking)

Engage and outreach to members with Serious Mental Illness/Substance Use Disorder, homelessness, and high medical needs within an interdisciplinary enhanced care model. Maintain a caseload using evidence-based engagement, conduct telephonic and SMS follow-ups, help build Health Action/Shared Care Plans, and coordinate referrals and appointments. Support nurse and behavioral health leaders with delegated tasks while consistently meeting encounter metrics and using tools like Google Suite, Slack, and CRM systems.
