Key Responsibilities

  • •Identify, outreach, engage, and enroll eligible individuals into the program virtually via phone/text/email and in-person at clinics and partner locations.
  • •Listen openly, meet people where they are, and support enrollment through multiple outreaches using empathetic communication.
  • •Educate partners about services and explain Enhanced Care Management benefits to eligible members, addressing initial hesitation.
  • •Use community knowledge and best-in-class customer service to build trust and enroll members into the program.
  • •Manage the end-to-end enrollment funnel, provide updates on delays/blockers, and meet enrollment metrics using internal and external technology tools.
