Key Responsibilities

  • •Provide comprehensive palliative care to patients in their homes, focusing on pain and symptom management, improving quality of life, and offering emotional and spiritual support.
  • •Perform in-depth assessments of patients with serious or life-limiting conditions, develop personalized care plans, and manage their medical needs.
  • •Work closely with patients and their families to establish goals of care and ensure treatment aligns with their wishes, emphasizing patient autonomy and dignity.
  • •Collaborate with a multidisciplinary team (nurses, social workers and other healthcare professionals) to ensure a holistic approach to patient care.
  • •Participate in a shared on-call rotation, as directed by the Senior Medical Director.

Key Requirements

  • •Physicians: MD or DO, Board Certified or Board eligible in Palliative Care, Hospice & Palliative Medicine, or related field; licensed in New Jersey or Georgia with DEA and CDS registrations (support provided if needed); minimum 3 years of clinical palliative care experience (home-based care and value-based care preferred).
  • •Nurse Practitioners: based in New Jersey or Georgia; BSN and Master’s in advanced clinical practice nursing (or post-Master’s certificate); active NJ NP license with DEA and CDS registrations; prior experience in inpatient palliative care or hospice nursing; house call experience; board certification in Palliative Care or Hospice Nursing preferred.
