Bilingual Case Management Specialist (Remote Flexible, Spanish Speaking)

Provide whole-person case management for individuals with serious mental illness, substance use disorder, and/or high medical needs. Maintain an ongoing caseload, use relationship-based outreach to support members, and conduct telephonic/SMS follow-ups. Help members set goals and incorporate them into Health Action/Shared Care Plans, coordinate community resources and appointments, and support interdisciplinary care teams through delegated tasks and case reviews. Remote-first role with occasional on-site community visits.
