Key Responsibilities

  • •Oversee up to 15 Collaborative Care Clinicians’ performance, including clinical metrics, schedule management, coaching, supervision, and performance management.
  • •Manage shift coverage and schedule needs across assigned practices, including approving time off and ensuring attendance policy adherence.
  • •Serve as a clinical and operational resource for complex patient situations, including escalated complaints, patient terminations, and incident reporting.
  • •Provide direct behavioral health care for a small caseload (no more than 15 patients), tailoring treatment plans and monitoring symptom changes and side effects.
  • •Document patient progress and recommendations in Salesforce and the electronic health record, track outcomes using a registry, and plan relapse prevention and mutual self-management goals.

Key Requirements

  • •Masters degree in social work, counseling, or related field.
  • •Licensed in California as an independent clinician (LCSW, LPCC, or equivalent).
