A frontline role within the Community Engagement Team fostering relationships with community stakeholders, healthcare providers, and referral sources to expand Amae Health’s network in the Raleigh, NC region. You’ll manage referrals, coordinate care transitions, advocate for the organization’s integrated care model, and track outreach and enrollment activities to meet targets in a fast-paced, mission-driven environment.

Key Responsibilities

•Actively manage and grow patient referrals in your region through systematic outreach to existing provider partners
•Identify and develop new potential referral sources to grow Amae’s presence and ensure a steady pipeline of patient referrals
•Coordinate rapid responses to urgent patient situations, collaborating with hospital systems, social workers, and providers to ensure timely care transitions
•Work closely with clinical staff to ensure that referred patients are connected with the appropriate care and support
•Serve as a visible, trusted ambassador for Amae Health in your community

Pay and Benefits

Salary: USD 60,000 - 84,000 annually

Key Requirements

•3+ years of client or customer-facing experience (e.g. in-person consulting, cold calling, digital outreach, business development, etc.), with defined engagement targets.
•Proven track record of building a referral base and relationships, such as business development, sales, community outreach, or referral management.
•Knowledge of, and passion for, mental health and wellness.
•Excellent oral and written communication skills, including strong public speaking ability.
•Current driver’s license, proof of acceptable automobile insurance coverage, and reliable transportation
