Key Responsibilities

  • •Oversee incoming Preferred Communities (PC) referrals, complete intake assessments, and enroll clients.
  • •Provide Intensive Case Management (ICM) by creating and implementing individualized goal plans that support long-term self-sufficiency.
  • •Deliver ongoing support through proactive client communication, guiding clients through healthcare, social service, and community systems.
  • •Maintain compliance by completing accurate documentation and ensuring timely, effective data collection and outcome tracking.
  • •Collaborate with program staff and the Volunteer Coordinator to coordinate wrap-around care and support; submit required reports and participate in trainings and meetings.
Travel: Low travel

Pay and Benefits

Salary: USD 22 - 23 hourly

Key Requirements

  • •Experience in case management, health education, or related fields; experience with refugee or vulnerable populations is preferred.
  • •A degree in Social Work, Public Health, or a similar field.
  • •Fluency in English is required.
  • •Bilingual in Arabic, Swahili, Spanish, French, Kareni, Burmese, Pashto, or Dari (as applicable).
  • •Strong ability to manage multiple cases, prioritize tasks, and deliver trauma-informed, culturally sensitive support.
