  • •Implement products and execute ACO initiatives with direction from Market Leadership.
  • •Accountably support wellness visits and transitions of care and ensure practices meet goals in mutually developed Practice Transformation Plans.

Key Requirements

  • •Work with training office staff and physicians/providers on Aledade’s value-based care processes and technologies.
  • •Assess a practice’s and staff’s training needs on an ongoing basis.
  • •3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes.
  • •Excellent oral and written communication skills, including public presentation, plus strong organizational and project management skills.
  • •4–6 years of relevant work experience in medical practice management (may substitute for education requirements), and familiarity with process mapping and workflow analysis tools.
