Key Responsibilities

•Provide high-quality, patient-centered care to residents in skilled nursing facilities.
•Conduct daily rounds, prescribe medications/treatment plans, and address acute/emergent issues.
•Communicate with facility nursing staff via phone or secure messaging as needed.
•Maintain reliable on-site attendance for scheduled shifts.
•Ensure compliance with clinical standards, facility policies, and regulatory requirements.

Key Requirements

•Active U.S. license and board certification by ANCC, AANPCB, PNCB, or AACN for NPs, or NCCPA for PAs; specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
•Experience providing patient-centered care in skilled nursing facilities or post-acute settings is preferred.
•Ability to conduct daily rounds, prescribe medications and treatment plans, and address acute/emergent issues.
•Proficiency with EHR documentation and charting; use of voice dictation and AI Scribe tools is a plus.
•Strong communication with facility staff and adherence to clinical standards and regulatory requirements.
